The mission of LTRCM is to assist individuals with severe physical and sensory disabilities to build a quality of life of their choosing through self-direction, support, and community resources.
The DARS Long Term Rehabilitation Case Management Services (LTRCM) Program assists people with severe physical and sensory disabilities to build a quality of life of their choosing through self-direction, support, and community resources. The LTRCM Program seeks to eliminate, reduce, or prevent economic and personal dependency. LTRCM Rehabilitation Specialists match an individual's rehabilitative needs with appropriate services and supports in the community.
LTRCM Rehabilitation Specialists provide specialized services to eligible individuals who need long-term case management services. Individuals with physical and sensory disabilities are often challenged by a complicated service delivery system as they seek assistance in meeting their needs. LTRCM Rehabilitation Specialists work collaboratively to plan services and build community supports to enhance an individual's quality of life, independence, and employment.
Coordination may include but are not limited to the exploration of available services and existing programs.
Individuals are eligible to apply for services through the DARS Long Term Rehabilitation Program (LTRCM) if they have a physical or sensory disability, require a special combination of life long or extended duration services, and their disability results in substantial limitations in three or more of the following life areas:
Long Term Rehabilitation Case Management Services are provided at no cost to individuals who meet disability and financial eligibility.
